1. All forms must be completed in full.
2. If you have all the requested information and you are relatively injury and illness-free, you may submit your paperwork upon arrival on campus.
3. If you have significant injuries or illnesses, you should send your documentation in advance as soon as possible so that we may evaluate your status and suggest further follow-up or testing.
